Why we still have safety issues with lithium batteries?

After 50 years of Lithium Battery Technology usage in research and development programs, and significant commercialization, we still continue to have safety incidents as the technology evolves and is used in new applications. This book was printed in1994. Today, April 16, 2013, I write the "first" review. Why we still have safety issues with lithium batteries? Sadly we do not learn and share our knowledge. I am including myself in this assessment. I could have referenced this book in a paper I wrote back in 2007. I made a mistake, which I hope I can start to correct post the GM Volt and Boeing 787 fires.
This book is a "must" for all serious professionals in the power sources community at government labs, battery manufacturers, research institutes, universities, systems integrators, etc. We can prevent the next safety incident by understanding the valve this reference book provides. It should have been a reference on the NTSB site not the wikipedia version of lithium-ion batteries.
